What is a Primary Care Network?

A primary care network is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.

Take a look at this short animation from NHS England which explains how they work.

Here to help you

The PCN is working towards making more services available for you such as later and weekend appointments, social prescribing and first contact physiotherapy. We are working in conjunction with health and social care services across Wolverhampton to provide you, your family and carers with improved access.

This means if you’re registered at one of our practices and you cannot get an appointment with your usual GP, you will be offered an alternative PCN appointment at one of the other GP practices.

Patient Participation Group

As a Trust we are committed to hearing patients’ views and opinions on the healthcare services we provide.

Our Patient Participation Group runs every month for patients to have their say and voice their concerns. If you are a member of any of the GP practices below, please join in!
